心脏MRI、心脏超声对慢性肺动脉高压心脏功能监测价值

摘    要:目的探究心脏磁共振成像(MRI)、心脏超声对慢性肺动脉高压患者心脏功能的监测价值。方法选择2016年7月至2018年2月我院确诊为慢性肺动脉高压的49例患者纳入研究。所有患者均行心脏MRI及实时三维超声扫描,比较两种方法测得左、右心室舒张末期容积(end-diastolic volume,EDV)、收缩末期容积(end-systolic volume,ESV)、每搏输出量(stroke volume,SV)、射血分数(ejection fraction,EF)等心功能参数的差异并分析其相关性。结果两种方法所测左心室EDV、ESV、SV、EF及右心室SV、EF等心功能指标差异无统计学意义(P>0.05);MRI所测右心室EDV、ESV显著高于心脏超声(P<0.05);Pearson相关分析示,心脏MRI及心脏超声所测左、右心室EDV、ESV、SV、EF均呈正相关(P<0.05)。结论实时三维超声及心脏MRI均对肺动脉高压患者左、右室心脏功能有较好的评估效果,但心脏超声在右心室容积方面的测定仍有待进一步提高。

Abstract:Objective To investigate the monitoring value of cardiac magnetic resonance imaging(MRI) and cardiac ultrasound in cardiac function of patients with chronic pulmonary hypertension. Methods 49 patients diagnosed with chronic pulmonary hypertension in the hospital from July 2016 to February 2018 were selected. All patients were given cardiac MRI and real-time three-dimensional ultrasound scan. The differences of cardiac function parameters such as left and right ventricular end-diastolic volume(EDV), end-systolic volume(ESV), stroke volume(SV) and ejection fraction(EF) measured by two methods were compared, the correlation was analyzed. Results There were no significant differences in cardiac function parameters such as EDV, ESV, SV, EF in left ventricular and SV and EF in right ventricular measured by the two methods(P>0.05). The EDV and ESV in right ventricular with MRI were significantly higher than those with cardiac ultrasound(P<0.05). Pearson correlation analysis showed that there were positive correlations in left and right ventricular EDV, ESV, SV and EF both measured by cardiac MRI and cardiac ultrasound(P<0.05). Conclusion Both real-time three-dimensional ultrasound and cardiac MRI have a good evaluation of left and right ventricular cardiac function in patients with pulmonary hypertension, the determination of right ventricular volume with cardiac ultrasound remains to be further improved.
