B超导引下腹腔穿刺对腹部闭合性外伤的诊断价值

目的探讨B超导引下腹腔穿刺对腹部闭合性外伤的诊断价值。方法对1990～2003年157例腹部闭合性外伤B超动态观察及B超导引下行腹腔穿刺。结果157例B超动态观察106例腹腔积液，超声导引下穿刺阳性106例。经手术证实与腹穿诊断肝、脾、肠系膜血管、胃及肠破裂符合率均为100％。结论穿刺所得物如为不凝血液，可疑为腹腔实质脏器或血管破裂，如涂片有脓细胞、白细胞，可考虑为空腔脏器破裂。

Diagnostic Value of Abdomen Puncture under the Guidance of B-type Ultrasound in Closed Abdomen Injury

Objective To investigate the diagnostic value of abdomen puncture under the guidance of B-type ultrasound in closed abdomen injury.Methods 157 patients with closed abdomen injury were observed dynamically and punctured under the guidance of B-type ultrasound from 1990 to 2003. Results 157 patients were observed,and abdomen fluid collections were found in 106 patients by B-type ultrasound.All of the 106 patients were positive by abdomen puncture under the guidance of B-ultrasound. The ruptures of liver, spleen, mesenteric vessels and gastrointesine were confirmed by operations with the percentage of 100%. Conclusion If uncoagulated blood is punctured out, solid organ or vessels rupture is suspected. If pus cells or leukocytes are found on the smear under microscope, hollow organ rupture can be considered.
