多层螺旋CT灌注成像在结直肠癌术后评价的应用探讨

目的探讨多层螺旋CT灌注成像在评价结直肠癌术后局部复发及瘢痕组织的血流动力学差异。资料与方法对17例经手术治疗后的结直肠癌患者进行多层螺旋CT灌注成像，其中术后复发10例，瘢痕组织7例，所有病例均经随访或病理证实。对所有病例选定靶平面，再进行同层动态增强扫描，并运用机器自带灌注软件测定局部复发病灶、瘢痕组织的血流量（BF）、血容量（BV）及计算平均通过时间（MTT）。结．豢结直肠癌术后复发的BF、BV值均高于瘢痕组织（P〈0．05），术后复发与瘢痕组织的MTT值差异无统计学意义（P〉0．05）。结论多层螺旋cT灌注成像能根据组织的血流动力学差异，帮助早期诊断结直肠癌术后复发病灶，有利于临床的跟踪治疗。

The Application of MSCT Perfusion Imaging in the Evaluation of Colorectal Cancer after Operation

Objective To investigate the hemodynamic characteristics of local recurrence of colorectal cancer and scar tissue after operation with MSCT perfusion imaging. Materials and Methods 10 cases of local recurrence and 7 cases of scar tissue were performed with MSCT perfusion imaging. All ases were proved by pathology or Iollow-up. Make sure the target slices in all cases, the dynamic enhanced scanning was taken in the target slices. To determinethe the BF,BV and MTT of local reccurence and scar tissue with the system comes with perfusion software. Results BF and BV values of the local recurrence were higher than those of scar tissue（P〈0.05）. MTT values were not significantly different between local recurrence and scar tissue（P〉0.05）. Conclusion The MSCT perfusion imaging can helps to make early diagnosis of the local recurrence according the difference of the hemodynamics in the tissue. It was benefit to the follow-up therapy.
