干细胞移植治疗失代偿期肝硬化临床评估

目的观察干细胞移植治疗失代偿期肝硬化的疗效。方法选择110例失代偿期肝硬化患者在常规治疗基础上行干细胞移植,移植后第4周复查肝功能、凝血酶原时间、胆碱酯酶和甲胎蛋白等实验室指标及观察临床症状和体征的变化。结果移植后第4周,血浆白蛋白（ALB）、胆碱酯酶（CHE）显著升高（P〈0.05或〈0.01）,凝血酶原时间（PT）显著下降（P〈0.05）,但天门冬氨酸氨基转移酶（AST）、丙氨酸氨基转移酶（ALT）、总胆红素（TBIL）和甲胎蛋白（AFP）水平在移植前后无显著性变化（P〉0.05）;干细胞移植可明显改善失代偿期肝硬化患者临床症状和体征（P〈0.05）,提高生存质量,术中未发生严重并发症,近期无不良反应出现。结论干细胞移植治疗失代偿期肝硬化有较好的近期疗效。

The therapeutic effect of stem cell transplantation on decompensated cirrhosis

Objective To observe the clinical efficiency of stem cell transplantation on decompensated cirrhosis.Methods 110 patients with decompensated cirrhosis,based on routine treatment,were treated by stem cell transplantation.The parameters including patients’ liver function,prothrombin time (PT),cholinesterase and alpha-fetoprotein were detected after 4-week treatment,and the changes of clinical symptoms of patients were observed.Results Four weeks after stem cell transplantation,the levels of plasma albumin (ALB) and cholinesterase were significantly increased (P<0.05 or P<0.01),however,the levels of PT was significantly decreased (P<0.05),furthermore,the levels of aspartate aminotransferase (ALT),alanine aminotransferase (AST) and total bilirubin (TBIL ) were not changed obviously (P>0.05) before and after stem cell transplant.The clinical symptoms,signs and life quality of patients with decompensated cirrhosis were significantly improved after stem cell transplantation (P<0.05).No adverse reactions and serious intraoperative complications were found in the near future.Conclusion The stem cell transplantation has better short-term therapeutic effect on decompensated cirrhosis.
