肺超声评分联合血清锌对儿童社区获得性肺炎预后判断的价值

目的 探讨与分析肺超声(lung ultrasound)评分联合血清锌对儿童社区获得性肺炎(community-acquired pneumonia，CAP)的预后判断的价值。方法选择儿童社区获得性肺炎72例作为研究对象，给予肺超声评分联合血清锌检查，随访患儿的预后并进行相关性分析。结果72例患儿随访时间为（13.09±0.28）个月，预后良好60例，占比83.3%。预后良好组的病程、年龄、性别、体重、身高与预后不良组对比差异无统计学意义(P>0.05)。预后良好组的肺超声评分明显低于预后不良组(P<0.05)，血清锌含量明显高于预后不良组(P<0.05)。在72例患儿中，Spearman分析显示肺超声评分、血清锌含量与预后良好都存在相关性(P<0.05)。多因素logistic回归分析显示肺超声评分、血清锌含量都为影响儿童社区获得性肺炎预后的重要因素(P<0.05)。结论儿童社区获得性肺炎预后不良的发生率比较高，多伴随有血清锌含量降低与肺超声评分升高，肺超声评分联合血清锌对儿童社区获得性肺炎的预后判断具有重要价值。

The prognostic value of lung ultrasound score combined with serum zinc in children with community-acquired pneumonia

Objective To investigate and analyze the prognostic value of lung ultrasound score combined with serum zinc in children with community-acquired pneumonia (CAP). MethodsSeventy-two children with community-acquired pneumonia who were diagnosed and treated in our hospital were selected. All of them were receivedlung ultrasound score combined with serum zinc examination. The prognosis of the children was followed up and correlation analysis was performed. Results All the children were followed up for (13.09±0.28) months, and 60 cases had good prognosis, accounting for 83.3%. There was no significant difference in the course of disease, age, gender, weight, or height between the good prognosis group and the poor prognosis group (P>0.05). The lung ultrasound score in the good prognosis group was significantly lower (P<0.05), and the serum zinc content was significantly higher (P<0.05). In 72 children, Spearsman analysis showed that lung ultrasound score, serum zinc content were significantly correlated with good prognosis (P<0.05). Multivariate logistic regression analysis showed that lung ultrasound score and serum zinc content were important factors affecting the prognosis of children with community-acquired pneumonia (P<0.05). Conclusion The incidence of poor prognosis in children with community-acquired pneumonia is relatively high, which is often accompanied by a decrease in serum zinc content and an increase in lung ultrasound score. Lung ultrasound score combined with serum zinc has important value in the prognosis judgment of children with community-acquired pneumonia.
