摘    要:目的 探讨不同生长激素分泌状态下矮身材儿童血脂水平的差异,为生长激素缺乏对儿童体脂代谢的影响提供理论依据。方法 收集矮身材儿童188例,依据生长激素药物激发试验峰值分为生长激素完全缺乏(cGHD)组、生长激素部分缺乏(pGHD)组、非生长激素缺乏性(nGHD)组,研究对象均禁食禁水10 h后空腹测定总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL)和高密度脂蛋白胆固醇(HDL)4项血脂水平。结果 3组儿童HDL水平比较差异无统计学意义(P>0.05);3组儿童TC、TG、LDL、non-HDL水平比较差异均有统计学意义(P<0.05);组间两两比较,cGHD组TC、LDL、non-HDL水平较其他组明显升高(P<0.05),cGHD组TG水平较nGHD明显升高(P<0.05),与pGHD组比较差异无统计学意义(P>0.05)。pGHD、nGHD组高TC、高LDL及高non-HDL的发生率明显低于cGHD组(P<0.05),nGHD组临界高LDL的发生率明显低于cGHD组(P<0.05)。而3组间TG、HDL的异常发生率及TC、TG...

Abstract:Objective To study the levels of blood lipid in short stature children with different growth hormone secretion status, so as to provide a theoretical basis for the effect of GH deficiency on body fat metabolism in children. Methods Totally 188 children with short stature were enrolled, and divided into completely growth hormone deficiency(cGHD) group, partial growth hormone deficiency(pGHD) group and non-growth hormone deficiency(NGHD) group according to the peak value of growth hormone drug stimulation test. Fasting venous blood was collectedto measure the levels of total cholesterol (TC), triglycerides (TG), low density lipoprotein cholesterol (LDL) and high density lipoprotein cholesterol (HDL). Results There was no significant difference in HDL among the three groups (P>0.05). There were significant differences in the levels of TC, TG, LDL and non-HDL among the three groups (P<0.05). The levels of TC, LDL and non-HDL in cGHD group were significantly higher than those in the other 2 groups (P<0.05), and the level of TG in cGHD group was significantly higher than that in NGHD group (P<0.05). There was no significant difference in TG level between cGHD group and pGHD group (P>0.05). The incidences of hypercholesterolemia, high LDL and high non-HDL in pGHD and NGHD groups were significantly lower than that in cGHD group (P<0.05), and the incidence of borderline-high LDL in NGHD group was significantly lower than that in cGHD group (P<0.05). There was no significant difference in the incidences of hypertriglyceridemia and low HDL,borderline-hypercholesterolemia, borderline-hypertriglyceridemia, borderline-low HDL or borderline-high non-HDL among the three groups (P>0.05). Conclusion The incidence of dyslipidemia in short stature children with different growth hormone secretion status increases in different degrees, especially in cGHD group. We should attach great importance to this in clinic and take relevant monitoring measures.
