斑点追踪技术评价成年白血病患者化疗前左心功

摘    要: 目的 探讨应用二维斑点追踪技术评价成年白血病患者化疗前左心整体及局部收缩功能的价值。方法 选择28例临床诊断的白血病患者及21名正常人,应用二维斑点追踪技术测量左心室长轴各节段收缩期纵向峰值应变(SL)、总体纵向峰值应变(GSL)、应变率(GSrL)、舒张早期及晚期总体纵向峰值应变率,短轴切面各节段收缩期圆周峰值应变(SC)及总体圆周峰值应变(GSC)、应变率(GSrC),舒张早期及晚期总体圆周峰值应变率。结果 白血病组各个节段的收缩期SL、SC及每个长轴、短轴切面的GSL、GSC、GSrL、GSrC、舒张早、晚期总体纵向及圆周峰值应变率均低于对照组。心尖段各组测值减低最为明显。结论 白血病患者化疗前左心室整体及局部收缩功能减低。二维斑点追踪技术可以快速准确地发现白血病患者用药前左心功能的减低,较常规二维测量值更加敏感。

Abstract:Objective To explore the value of two-dimensional speckle tracking imaging (2D-STI) in evaluating left ventricular function in adult patients with leukemia before chemotherapy. Methods Totally 28 leukemia patients and 21 normal control subjects were enrolled. The longitudinal and circumferential peak strain (SL, SC) of every segment, the global longitudinal and circumferential peak strain (GSL, GSC), global longitudinal and circumferential peak strain rate (GSrL, GSrC), the early and later peak diastolic strain rate of every long axis section and every short axis section were measured. Results Compared to control group, the SL, SC of every segment and GSL, GSC, GSrL, GSrC, the early and later diastolic peak strain rate of every long axis and short axis section of leukemia patients were lower, and the decline was more significant at apex segment. Conclusion Global and regional left ventricular function in patients with leukemia before chemotherapy are abnormal. Systolic function of leukemia patients before chemotherapy can be early, accurately evaluated with 2D-STI, which is more sensitive than two-dimensional measurements.
