乳头状瘤病毒与人类肿瘤

摘    要:本课题组近3年来,以食管癌为主,用PCR、RNA原位杂交及免疫组化的方法检测了人类乳头状瘤病毒(HPV)在肿瘤组织及癌前病变中的存在,发现在食管癌、乳腺癌和卵巢癌中均有很高比例的HPV检出率(63%、66%及50%)以及活跃的病毒mRNA或蛋白表达.在食管癌高发人群的普查标本上,随着食管癌上皮癌前病变程度的加重,HPV检出率和表达率逐渐增高.我们同时发现选择不同的病毒靶位开展PCR会得到不同的检出率.E6/E7片段可较真实地反映HPV感染情况. 我们的结果提示:HPV-16与食管癌、乳腺癌和卵巢癌的发生密切相关;HPV DNA的存在和活跃的表达与细胞癌变相关;HPV-16E6/E7与L1片段相比,检出率大大提高.我们认为HPV与人类肿瘤的关系应该受到更多的关注,结合对HPV感染的人群筛选遗传易感基因,对高危人群识别、预防、早诊和治疗,以降低肿瘤的发病率具有重要意义.

Human papilloma virus (HPV) and Human Cancers
Abstract:For the last 3 years, we have human papilloma virus-16 (HPV-16) tested for its infection in esophageal (30), breast (61) and ovarian (50) cancers by PCR and m RNA in situ hybridization (ISH) and immunohistochemistry (IHC) of L1, E6 and E7 respectively. High infection rate has been found with 63%, 66% and 50% in the above cancers r espectively. Most of the positive samples also showed active HPV-16E6 expressio n by ISH and IHC. In the cytological samples of esophageal epithelium from high i ncident population, a progressive increased positive rate along with the patholo gical severity has been observed. We have also found that positive rate would be diff erent when targeting different fragment of the virus in our PCR assay and E6/E7 were suitable for the assay. Our results suggest that HPV-16 is closely related with carcinogenesis of esophageal, breast and ovary cancers; HPV-16E6/E7 instea d of L1 fragment have a high test rate. We think that more attention should be paid to the relationship between HPV and human cancers. Screening of genetic sus cep tible factors against HPV infection will be contributive to the iden tification of high risk individual, prevention, intervention and early diagnosis of these tumors.
