摘    要:目的 应用脉冲组织多普勒技术评价尿毒症患者右室整体功能及负荷状态.方法 随机选取行透析治疗的尿毒症患者20例(尿毒症组)及30例健康体检者(对照组).采用脉冲组织多普勒成像获取心尖四腔切面右室侧壁三尖瓣环运动频谱,测量等容收缩时间(ICT)、射血时间(ET)、等容舒张时间(IRT)、收缩期峰值运动速度(St)、舒张早期峰值运动速度(Et)、舒张晚期峰值运动速度(At),计算Et/At及Tei指数.结果 与对照组比较,尿毒症组右室ET缩短,IRT延长,Et减低,At增加,Et/At降低,Tei指数增大,差异均有统计学意义(P <0.05).结论 尿毒症患者右室整体功能异常,主要为舒张功能受损,参数的变化情况正确地反映了右室压力和容量超负荷状态.

Abstract:Objective To evaluate right ventricular function and load condition in patients with uremia by pulsed wave tissue Doppler imaging(PW-TDI).Methods The spectrum of tricuspid ring in right ventricular lateral wall were acquired in apical four chamber view by PW-TDI in 20 patients with uremia and 30 healthy volunteers.The isovolumic contraction time(ICT),ejection time(ET),isovolumic relaxation time(IRT),peak systolic velocity(St),peak early diastolic velocity(Et) and peak late diastolic velocity(At) were measured,Et/At and Tei index were calculated.Results Compared with control group,ET shortened,IRT extended,Et decreased,At increased,Et/At reduced and Tei index increased in uremia groups,there were significant difference(P <0.05).Conclusion The global function of right ventricular in patients with uremia shows abnormal and diastolic dysfunction is dominant.The change of PW-TDI parameters can reflect correctly the pressure and volume overload conditions of right ventricular in patients with uremia.
