妊娠期念珠菌性阴道炎的病原鉴定、药敏分析和治疗

目的:探讨妊娠期念珠菌性阴道炎的病原体分布、体外药敏结果和初步临床治疗效果。方法:对160株孕妇感染标本中分离出的念珠菌进行鉴定,进行药敏分析;对其中可随访的60例患者进行阴道置入制霉菌素制剂治疗。结果:白色念珠菌的检出率最高(60.6％);体外药敏分析显示:对各种念珠菌的总体敏感率大于90％的抗真菌药物有制霉菌素(94.4％)、伊曲康唑(93.8％)、两性霉素B(93.8％)、5-氟胞嘧啶(91.9)。制霉菌素制剂的治疗总有效率93.3％,与体外药敏结果(94.4％)基本一致。结论:妊娠期念珠菌性阴道炎以白色念珠菌感染为主,临床治疗可首选制霉菌素,可考虑选用两性霉素B、伊曲康唑、5-氟胞嘧啶等药物。

Identifications and Drug Sensitivity of Pathogeny and Clinical Therapy of the Pregnant Monilial Vaginitis

Objective: To discuss the distributions and drug sensitivity of pathogeny, and clinical therapeutic efficacy of the pregnant monilial vaginitis. Methods: To identify the 160 monilia from 160 patients ,and the bacteria's drug susceptibility test were preformed.Among them,60 patients were treated with nystatin.Results: Total of 160 strains pathogens were isolated from 160 pregnant women from January to September in 2002,among them 97strains were Candida albicans (97/160,60.6%). The total percentage of susceptibility of Monilia to 5-fluorocytosine, amphotericin B , nystatin and itraconazole were 91.9% ,93.8% , 94.4% and 93.8%, respectively . The total therapeutic efficacy of nystatin for the pregnant monilial vaginitis was 93.3%. Conclusions: The major pathogeny of the pregnant monilial vaginitis was Candida albicans,the first choice antibiotics for clinical therapy was nystatin,5-fluorocytosine, amphotericin Band itraconazole also can be used.
