摘    要:目的比较梅毒检测的甲苯胺红不加热血清试验(TRUST)和酶联免疫吸附试验(ELISA)两种方法的敏感性和特异性,选择一种更适合大批量母血和脐血标本的梅毒筛查方法。方法选择2007年6月~2007年12月天津市脐带造血干细胞库的母血和脐血标本共8005份(16010对),同时用ELISA和TRUST方法进行检测,对任意一种方法检测出的阳性标本再用梅毒螺旋体颗粒凝集试验(TPPA)的方法进行确认。结果TRUST法的阳性检出率是0.33%(52/16010),ELISA法的阳性检出率是0.55%(88/16010)。52份TRUST法检测为阳性的标本中经TPPA确认为阳性的有41份,有11份为假阳性;88份EUSA法检测为阳性的标本中经TPPA确认为阳性的有87份,有1份为假阳性,(P〈0.05)。结论用ELISA作为检测脐血梅毒的初筛方法,能够确保临床脐血干细胞移植的安全。

Comparisons of ELISA and TRUST Method in Detecting the Antibody of Syphilis

Abstract:Objective To compare the sensitivities and specificities of two methods of syphilis detection by way of TRUST and ELISA in order to find a better syphilis screening method for a large amount of maternal and umbilical cord blood samples. Method: 8005 samples (16010 pairs) of maternal and umbilical cord blood collected and preserved in Tianjin Umbilical Cord Blood Bank from June, 2007 to December, 2007 were examined both by TRUST and ELISA and then the detected positive samples were further examined by treponema pallidum particle assay (TPPA). Results Positive detectable rate of TRUST is 0.33%, whereas ELISA is 0.55%. Among 52 positive samples (0.33% of all the samples) confirmed by TRUST, 41 samples were positive and 11 samples were false positive by TPPA. Among 88 positive samples (0.55% of all the samples) confirmed by ELISA, 87 samples were positive and one was false positive by TPPA. Conclusion ELISA can be applied as a primary screening method of syphilis detection in umbilical cord blood to ensure the safety of clinical cord blood stem cell transplantation.
