原发性胆囊癌28例临床分析

目的 探讨原发性胆囊癌的诊断.方法 回顾性分析该院2002年4月至2007年8月收治的经手术及病理证实的胆囊癌28例.结果 主诉为右上腹疼痛24例,黄疽3例.术前确诊率96.43%.合并胆囊结石12例,占42.86%.Ⅱ期1例已生存4年尚无复发.1年生存率为38.46%.结论 原发性胆囊癌临床症状无特异性,影像学检查很重要,加强对胆囊癌诊断的意识及对易感人群进行超声筛查并结合CA199、CEA等肿瘤标志物分析对提高早期诊断率是必要的.

Clinical analysis of 28 primary gallbladder carcinoma cases

Objective To search the effective approach of diagnosis for primarygallbladder carcinoma.Methods A retrospective analysis on clincal pathological data was made for 28 cases of primary gallbladdercarcinoma treated in our hospital from April,2002 to August,2007.Results The presenting symptom of 24 patients was right upper abdominal pain and chief complaint of 3 patients was jaundice.The accurate diagnosis rate preoperative was 96.43%.42.86%of the patients was accompanied with gallstone.In this study,the long-term survivor was in Nevin's Stage Ⅱ.The 1-year overal survival rate was 38.46%.Conclusion The primary gallbladdercarcinoma has no typical symptom.The check of imageology is very important.It is important for enhancing early diagnosis rate to pay more attention to the exist of gallbladdercarcinoma and screen patients in acceptable crowd by combining B-mode ultrasonography with tumor markers, such as CA199,CEA,and so on.
