OCSP分型在急性缺血性卒中的临床应用

目的:分析急性缺血性卒中患者OCSP分型的构成,结合不同亚型的NIHSS评分,探讨不同亚型与神经功能缺损程度的关系.方法:对173例急性缺血性卒中患者进行美国国立卫生研究所卒中量表(NIHSS)评分,并按照OCSP的分型标准分为4大亚型,分析不同亚型与NIHSS评分的关系.结果:本组患者OCSP各亚型的构成为:腔隙性梗死36.42％,部分前循环梗死31.21％,后循环梗死17.92％,完全前循环梗死14.45％.4个亚型中,完全前循环梗死NIHSS评分最高、神经功能缺损程度最高,其次为后循环梗死和部分前循环梗死,腔隙性梗死NIHSS评分最低.4种类型的NIHSS评分差异有统计学意义(P＜0.05).结论:OCSP分型方法可用于对急性缺血性卒中患者进行临床分析,神经功能缺损程度随OCSP不同亚型而变化,OCSP分型可用于评价急性缺血性卒中患者的神经功能缺损程度.

OCSP classification in the clinical application of acute ischemic stroke

Objective: To investigate the proportion of ischemic stroke subtypes according to the Oxfordshire Community Stroke Project(OCSP),Combined with different subtypes of NIHSS scores,then the correlation between subtypes with neurological function impairment was analyzed.Methods:173 registered consecutive ischemic stroke inpatients were assessed neurological function by respectively the National Institute of Health Stroke Scale(NIHSS),and classified into 4 groups according to the OCSP criteria,then analysis the correlation between the score and OCSP subtypes.Results:The proportion of the different ischemic stroke subtype in 173 patients was as follows: lacunar infarcts was 36.42%,partial anterior circulation infarcts was 31.21%,posterior circulation infarcts was 17.92%,total anterior circulation infarcts was 14.45%,and among 4 stroke subtypes,total anterior circulation infarcts patients had the highest NIHSS score,next is posterior circulation infarcts and partial anterior circulation infarcts,Lacunar infarcts subtypes were associated with the lowest levels of NIHSS score.Four types of NIHSS scores are statistically significant differences(P<0.05).Conclusion:OCSP classification can be used in acute ischemic stroke patients for clinical analysis,nerve function defect changed with different OCSP subtypes,OCSP classification could be applied to evaluate nerve function defect with acute ischaemic stroke patients.
