利多卡因氧气雾化加清醒镇静麻醉在纤维支气管镜引导困难气管插管中的应用

目的观察利多卡因氧气雾化加清醒镇静麻醉用于纤支镜引导困难气管插管的效果。方法60例困难气管插管，随机分成两组，每组30例。观察组采用2％利多卡因氧气为动力雾化，对照组用常规喷雾＋环甲膜穿刺注药。两组均在局麻成功后静注咪唑安定0．05mg／kg、芬太尼2ug／kg。结果两组麻醉都能用于纤支镜引导困难气管插管。观察组麻醉效果优于对照组，两组相比P〈0．05。观察组各时点血压、SPO2无明显变化。HR在窥镜入声门、插管即刻时较雾化前有轻度增快，P〈0．05。结论2％利多卡因氧气雾化加清醒镇静麻醉引导纤支镜困难气管插管效果好，简便安全易行，可以广泛应用于临床。

Aerosolized lidocaine and conscious sedation anesthesia in difficult endotracheal intubation leading by fiberoptic bronchoscopy of the application

Objective To investigate results of aerosolized lidocaine and conscious sedation anesthesia in difficult endotracheal intubation leading by fiberoptic bronchoscopy. Methods 60 cases of difficult endotracheal intubation were randomly divided into two groups,each of 30 cases. The observed group, lidocaine 2% oxygen-driven atomization,and the control group received conventional anaesthesia method of spraying with circothyroid embrane puncture. Both groups were successful in local anesthesia after intravenous midazolam 0.05mg/kg, fentanyl 2ug/kg. Results Anesthesia was able to guide the bronchoscopy difficult tracheal intubation. The nacrotic effects of observed group was better than the control group,P<0.05. Observed group of the de facto blood pressure, SPO2 no significant changes. HR Tone endoscope in the door,intubation time to spray immediately before a slightly faster,P<0.05. Couclusion Aerosolized lidocaine 2% and conscious sedation anesthesia in difficult endotracheal intubation leading by fiberoptic bronchoscopy effective,simple,safe,easy and can be widely used in clinical practice.
