超声对胰腺损伤的诊断价值

目的探讨超声诊断胰腺损伤的价值。方法对经手术或CT证实的16例胰腺损伤的超声检查做回顾性分析。结果16例胰腺损伤中：挫伤4例,不完全断裂5例,完全断裂7例,其中11例显示超声直接征象,包括腺体局部回声减低、中断、假性囊肿等;13例显示小网膜囊积液等间接征象;漏诊3例。结论超声检查能准确判断胰腺损伤的位置、范围、程度及合并其它脏器的损伤,为临床手术治疗提供有力的依据。

Ultrasonographic Diagnosis of Pancreatic Trauma

Objective To explore the ultrasonographic findings and diagnostic value of pancreatic injury. Methods Abdominal ultrasonographic findings were reviewed in 16 patients proved pancreatic trauma by operation in 12 cases and CT/MRI in 4 cases. Results In 16 patients, there were 4 patients with pancreatic contusion, 5 with pancreatic incomplete broken and 7 with pancreatic complete broken. The direct ultrasonographic signs of pancreatic trauma were found in 11 patients, including the focal low-echoic zone, the broken and irregularly pancreatic contour, etc. The indirect signs such as the fluid in lesser omental bursa were found in 13 patients. There were 3 missed diagnostic cases mainly due to indistinct images. Conclusions The position, degree and coexistent injury of pancreatic trauma could be detected exactly on ultrasonography which may provide rapid and reliable bases for clinical surgery.
