局麻无张力疝修补术在老年腹股沟复发疝中的应用

目的 探讨局部麻醉无张力修补老年腹股沟复发疝的临床应用价值.方法 2001年4月～2005年9月,在局部神经阻滞麻醉下,采用无张力疝修补术治疗老年腹股沟复发疝患者65例,其中疝环填充法48例,三合一补片法17例.观察手术时间、切口疼痛、术后功能恢复、并发症及复发率.结果 手术全部成功完成,平均手术时间45 min.术后无尿潴留等相关麻醉并发症.切口皮下血肿2例,发生率3.08%.术后5～7 d出院,随访6～66个月,随访率95.38%.复发1例,复发率1.54%.结论 局部神经阻滞麻醉下无张力修补术是治疗老年腹股沟复发疝的理想术式,具有手术安全、创伤痛苦小、术后恢复快、疗效满意、并发症少和复发率低等优点.

THE USE OF LOCAL ANAESTHESIA FOR TENSION-FREE HERNIORRHAPHY IN ELDERLY INGUINAL RECURRENT HERNIA

Objective To evaluate the value of local herniorrhaphy in elderly patients with inguinal recurrent hernia. 9,65 cases of elderly inguinal recurrent hernia were treated with t local nerve blockade anaesthesia, including mesh plug-patch in 48 anaesthesia for tension-free Methods From 2001.4 to 2005. ension-free herniorrhaphy under cases and prolene system in 17 cases. The average postoperative stay was 5-7 days. The follow-up period ranged from 6 to 66 months and the follow-up rate was 95.38%. One case was recurred and the recurrence rate was 1.54%. Conclusion Tension-free herniorrhaphy under local nerve blockade anaesthesia is a perfect surgical operation for the elderly inguinal recurrent hernia. It has many advantages, such as safe, minimal invasion, less postoperative pain, rapid recovery, better recent therapeutic effect, less complications, and lower recurrence.
