人工全髋关节置换治疗成人先天性髋关节发育不良继发骨关节炎的效果分析

目的探讨人工全髋关节置换手术在成人先天性髋关节发育不良继发髋关节骨性关节炎外科治疗中的作用。方法10例(12髋)先天性髋关节发育不良继发骨性关节炎患者按Crowe分级,其中Ⅰ型5例6髋,Ⅱ型3例3髋,Ⅲ型1例1髋,Ⅳ型1例2髋,行全髋关节置换术。结果患者术后无神经血管损伤、切口感染及早期人工关节脱位等并发症。所有患者术后疼痛完全消失,髋关节活动度增加,随访Harris评分由术前平均(42.0±1.8)分提高到(89.0±2.2)分,与治疗前比较差异有统计学意义(t=2.201,P<0.05)。结论人工全髋关节置换手术是治疗成人先天性髋关节发育不良继发骨性关节炎的有效方法之一。

Curative Effect Analysis of Congenital Hip Dysplasia in Adult with Total Prosthetic Hip Replacement

Objective This article is written to evaluate the effectiveness of total prosthetic hip in replacing osteoarthritis secondary to congenital hip dysplasia. Methods 10 cases（ 12 hip） of osteoarthritis secondary to congenital hip dysplasia are treated with total prosthetic hip replacement. Among them, there are 5 cases （6 hip） of type Ⅰ ,3 cases （3 hip） of type Ⅱ ,one case of type Ⅲ and one case （2 hip） of type Ⅳ according to Crowe classification. Rresults Patients have no complications, such as nerve and blood vessel injury, infection of incisional wound and early dislocation of artificial joint. In all of the cases, pain is vanished and motion of hip-joint is increased after operation. By following up, Harris score increases from （42 ± 1.8 ）score to （89 ± 2.2 ） score in average after operation and there is obvious difference compared before operation （ t = 2. 201, P 〈 0.05 ）. Conclution It is concluded that total prosthetic hip replacement is one of the effective methods to treat osteoarthritis secondary to congenital hip dysplasia.
