基底细胞样型乳腺癌的临床形态特征及预后分析

目的：观察基底细胞样乳腺癌组织的特异性表达，探讨其临床形态特征，治疗效果如何及预后情况。方法：分析78例乳腺癌患者的临床资料，对其癌组织行病理观察、免疫组化（EnVision法）检测，分析基底细胞样乳腺癌所占的比例，观察患者预后情况。结果：78例患者均为女性，年龄27～78岁，平均49岁。EGFR表达阳性细胞平均比率为48．4％（59／120）。CK5／6旧性细胞平均比率为62．5％（75／120）。基底细胞样乳腺癌的复发率为89％，高于其他类乳腺癌2．6％（2／78）。死亡率在本组78例患者中基底细胞样乳腺癌为2．6％（2／78），非基底细胞样乳腺癌为1．3％。但基底细胞样乳腺癌在本类疾病中的死亡率为9．5％（2／21），明显较高。结论：基底细胞样型乳腺癌在乳腺癌中的发病率较高，其EGFR和CK5／6的高表达率为诊断该疾病提供依据，基底细胞样型乳腺癌患者的放化疗，内分泌治疗等效果不佳，预后差。

Clinical characteristics and prognosis analysis of the basal-like breast cancer

Objective: To observe the basal-like breast cancer tissue-specific expression,and explore the clinical morphological characteristics,treatment effectiveness and prognosis.Methods: We analyzed 78 cases clinical data of breast cancer patients by immunohistochemistry(EnVision method),and observed the proportion of cancer tissues and the patients prognosis.Results: 78 patients were all female with the aged by 27 to 78 years,average 49 years old.The average ratio of positive CK5/6 cells was 62.5%(75/120).The recurrence rate of basal-like breast cancer which was 8.9 percent was higher than that for other types of breast cancer: 2.6%(2/78).The mortality of basaloid breast cancer in this group of 78 patients was 2.6%(2/78),while that of non-basal-like breast cancer was 1.3%.The mortality rate of was Basal-like breast cancer in this class of diseases 9.5%(2/21),significantly higher.Conclusion: The incidence of basal cell-like breast cancer in the breast cancer was higher.The expression of EGFR and CK5/6 was to provide a basis for diagnosing the disease.And the prognosis of basal-like breast cancer patients was poor with radiotherapy and chemotherapy,endocrine therapy.
