高频超声与X线钼靶联合应用对早期乳腺癌的诊断价值

摘    要:目的探讨高频超声与X线钼靶联合应用对早期乳腺癌的诊断价值。方法分别采用高频超声与X线钼靶两种方法检查61例乳腺小肿块,观测病变大小、内部回声及血流等,全部病例经穿刺活检或手术病理证实。结果高频超声检查的敏感性、特异性、准确性、阳性预测值、阴性预测值及假阳性率、假阴性率分别是82.61%、53.33%、75.41%、84.44%、50.00%及46.67%、17.34%;钼靶分别是89.13%、60.00%、81.97%、87.23%、64.29%及40.00%、10.87%,两种方法比较无显著性差异。二者联合应用的各指标分别是95.65%、73.33%、90.16%、91.67%、84.12%及26.67%、4.35%,准确性比单一方法有明显提高,而假阳性率和假阴性率显著降低。结论两种方法联合应用能提高早期乳腺癌诊断的敏感性及准确性,降低误诊率和漏诊率,具有较高的临床实用价值。

Abstract:Objective To assess the value of high frequency ultrasound combined with mammography in the diagnosis of early breast cancer. Methods Sixty-one female patients who were examined by the high frequency ultrasound as well as mammography and confirmed by pathological examination. The size, internal echo and blood signals were observed in all patients. Results The sensitivity, specificity, accuracy, positive predictive value, negative predictive value and false positivity rate, false negativity rate of high frequency ultrasound and mammography were 82.61%, 53.33%, 75.41%, 84.44%, 50.00%, 46.67%, 17.34% and 89.13%, 60.00%, 81.97%, 87.23%, 64.29%, 40.00%, 10.87%, respectively. There was no significant difference between them. The accuracy of combining two methods was significantly higher than that of only one, the sensitivity, specificity, accuracy, positive predictive value, negative predictive value could reach 95.65%, 73.33%, 90.16%, 91.67%, and 84.12%, while false positivity rate (26.67%) and false negativity rate (4.35%) decreased. Conclusion Combined two methods can increase the diagnostic sensitivity and accuracy of early breast cancer, and has the significant value of clinical application.
