Clinical and pathological features of idiopathic portal hypertension

Objective To investigate the clinical and pathological features of idiopathic portal hypertension.Methods The clinical and pathological features of 16 patients with idiopathic portal hypertension were retrospectively studied and compared.Results 8 patients were males and females,respectively,with age averaged(34.2±14.7) years,and illness duration 1 month to 14 years.There were 10 cases with splenomegaly and hypersplenism,6 cases with esophageal varices,5 cases with upper digestive tract bleeding and malena,7 cases with anemia.9 cases with normal liver function and 7 cases with abnormal liver function.8 cases with CHE lower than 6000U/L had more severe portal hypertension but had milder pathological findings.There were 8 cases with cirrhosis on B type ultrasound and CT scan.These patients had higher ALT,AST and TBil lever and lower CHE level,compared with those without cirrhosis.The pathological findings,however,had no significant differences the severity of portal hypertension and pathological findings were mider in 6 cases with collateral circulation,compared with other patients.Conclusions The diagnosis of idiopathic hypertension should be considered in patients with portal hypertension of unknown origin and normal liver function and the pathological features were of diagnostic.
