摘    要:目的观察抑制视可尼喉镜气管插管反应,靶控输注不同浓度瑞芬太尼对丙泊酚半数有效浓度(EC50)的影响。方法择期全麻耳鼻喉科手术患者68例,ASA Ⅰ~Ⅱ级。依据瑞芬太尼靶浓度不同随机分为4组:RF1.5组,1.5ng/ml;RF2.0组,2.0ng/ml;RF2.5组,2.5ng/ml;RF3.0组,3.0ng/ml。同时靶控输注丙泊酚,以序贯法确定丙泊酚EC50,并计算95%可信区间。结果 RF1.5、RF2.0、RF2.5、RF3.0组视可尼喉镜插管无反应的丙泊酚EC50分别为3.62、3.38、2.90、2.69μg/ml。结论视可尼喉镜插管刺激强度小于传统喉镜,瑞芬太尼靶控浓度为2.0~2.5ng/ml时,丙泊酚EC50为3.38~2.90μg/ml,是较适宜的插管浓度。

Abstract:Objective To determine the medium effective concentration(EC50) of propofol based on the target controlled infusion(TCI) of remifentanil in the Shikani Optical Stylet(SOS) tracheal intubation.Methods Sixty-eight ASA Ⅰ~Ⅱ patients underwent ENT surgery were studied.Anesthesia induced both propofol and remifentanil given by TCI.The target effect concentration of remifentanil was set at 1.5、2.0、2.5、3.0 ng/ml.The EC50 of propofol was determined by up and down sequential trial.Results The EC50 of propofol was 3.62、3.38、2.90、2.69 μg/ml when remifentanil was 1.5、2.0、2.5、3.0 ng/ml.Conclusion The optimal target concentration for SOS intubation is 2.0~2.5 ng/ml for remifentanil and 3.38~2.90 μg/ml for EC50 of propofol.
