重庆地区376例胃癌手术病例分布特征及手术疗效比较

目的分析重庆地区胃癌住院患者的分布特征，比较腹腔镜与开腹手术的疗效，为完善胃癌防治策略提供科学依据。方法收集第三军医大学附属西南医院2005年1月-2010年12月收治的全部胃癌患者病历资料，以病人ID号码采集病人个人特征、临床特征和手术治疗选择等信息，并进行流行病学分布特征描述；根据手术方式的不同进行围手术期疗效指标的比较。结果患者来源于重庆市区及周边地区，共计64个区县，其中沙坪坝区最多；男女比例2．24：1，51—60岁胃窦癌发病率最高；腹腔镜手术和开腹手术比较，前者术中出血量少，但手术时间会延长。结论重庆市胃癌住院病例在病例来源、发病部位等方面具有一定的分布规律，腹腔镜相比开腹手术有优势，可作为临床优先选择的术式。

A comparison of the distribution characteristics and different surgery methods of 376 patients with stomach cancer

Objective To study the distribution characteristics of stomach cancer in Chongqing and to compare the clinical parameters between laparoscopic surgery and open surgery, so as to provide scientific basis for prevention and control strategy. Methods The clinical data of 376 cases of stomach cancer who were hospitalized in Southwest Hospital were collected and reviewed retrospectively from January 2005 to December 2010. Results Through the study, the results showed that the patients from 64 local areas （mostly from Shapingba District） at the age of 51 to 60 accounted for the largest proportion of gastric antral cancer. And the majorirty of these patients need surgery. Compared with the traditional open surgery, laparoscopic surgery was of less intraoperative blood loss but longer duration of operation. Conclusion The distribution of stomach cancer in Chongqing has its special characteristics, and laparoscopic surgery should he the first choice for stomach cancer.
