醋酸曲普瑞林联合激素替代治疗对多囊卵巢综合征不孕症患者IVF-ET临床效果的影响观察

目的探究醋酸曲普瑞林联合激素替代治疗对多囊卵巢综合征(PCOS)不孕症患者体外受精-胚胎移植(IVF-ET)临床效果的影响。方法选取2018年6月至2019年6月河南省生殖妇产医院收治的82例行IVF-ET的PCOS患者为研究对象,使用随机数字表法分为对照组和观察组,各41例。对照组患者予以激素替代治疗,观察组患者在对照组基础上联合醋酸曲普瑞林进行治疗。比较两组患者治疗前、治疗后的血清性激素水平促黄体生成素(LH)、雌二醇(E2)、孕酮(P)]、子宫内膜厚度差异,比较两组患者卵泡情况(获得卵泡个数、卵母细胞成熟率)及受精结果(受精率、优质胚胎率)、妊娠结局(妊娠率、流产率)差异。结果治疗后,两组患者LH、E2均较治疗前降低,P水平均较治疗前升高(P<0.05);治疗后,两组患者LH、P水平差异无统计学意义(P>0.05),观察组E2水平低于对照组(P<0.05);治疗后,两组患者子宫内膜厚度均较治疗前升高,且观察组高于对照组(P<0.05);观察组患者获取卵泡个数高于对照组(P<0.05),两组患者卵母细胞成熟率、受精率、优质胚胎率差异无统计学意义(P>0.05);观察组患者妊娠率高于对照组,而流产率低于对照组(P<0.05)。结论醋酸曲普瑞林联合激素替代治疗有助于改善PCOS不孕症患者IVF-ET的血清性激素水平,增加患者子宫内膜厚度,有利于患者获取卵泡数增加,并改善患者的妊娠结局。

Influence of triptorelin acetate combined with hormone replacement therapy on clinical effects of IVF-ET in infertility patients with polycystic ovary syndrome

【Objective】To explore the influence of triptorelin acetate combined with hormone replacement therapy on clinical effects of in vitro fertilization-embryo transfer(IVF-ET)in infertility patients with polycystic ovary syndrome(PCOS).【Methods】Eighty-two patients with PCOS who underwent IVF-ET in our hospital from June 2018 to June 2019 were selected as the research subjects,and they were divided into control group and observation group by using random number table method,with 41 cases in each group.Patients in control group were given hormone replacement therapy,and patients in observation group were combined with triptorelin acetate on the basis of control group.The serum sex hormones levelsluteinizing hormone(LH),estradiol(E2),progesterone(P)]and endometrial thickness were compared between the two groups before and after treatment.Differences in follicle conditions(the number of obtained follicles,maturation rate of oocytes),fertilization results(fertilization rate,high-quality embryo rate)and pregnancy outcomes(pregnancy rate,abortion rate)were compared between the two groups.【Results】After treatment,the levels of LH and E2 in the two groups were lower than those before treatment while the P level was higher than that before treatment(all P<0.05).After treatment,there were no significant differences in the levels of LH and P between the two groups(all P>0.05),and the E2 level in observation group was lower than that in control group(all P<0.05).After treatment,the endometrial thickness in the two groups was higher than that before treatment,and the endometrial thickness in observation group was higher than that in control group(all P<0.05).The number of obtained follicles in observation group was higher than that in control group(P<0.05).There were no significant differences in the maturation rate of oocytes,fertilization rate and high-quality embryo rate between the two groups(all P>0.05).The pregnancy rate in observation group was higher than that in control group while the abortion rate was lower than that in control group(P<0.05).【Conclusion】Triptorelin acetate combined with hormone replacement therapy can help improve the serum sex hormones levels of PCOS patients with infertility undergoing IVF-ET,increase the endometrial thickness,help increase the number of obtained follicles,and improve the pregnancy outcomes of patients.
