摘    要:目的探讨红细胞输注对早产儿脑氧代谢及生命体征的影响。方法选取160例早产儿,按照随机数表法分为研究组与对照组各80例,对照组患儿予以常规治疗,研究组在对照组基础上联合红细胞输注。比较治疗前及治疗后2 h时,两组患儿脑氧代谢脑组织氧饱和度(CrSO2)、氧摄取分数(CFOE)]水平变化;比较输血前及治疗后2 h时,两组患儿生命体征心率(HR)、收缩压(SBP)、舒张压(DBP)、平均动脉压(MABP)、经皮动脉氧饱和度(TcSaO2)]水平变化。结果治疗后2 h时,研究组患儿CrSO2水平均较治疗前升高(P<0.05),且明显高于同一时间对照组(P<0.05),研究组患儿CFOE水平均较治疗前降低(P<0.05),且明显低于同一时间对照组(P<0.05);治疗后2 h时,研究组患儿HR水平均较治疗前降低(P<0.05),且明显低于同一时间对照组(P<0.05),DBP、MABP水平均较治疗前升高(P<0.05),且明显高于同一时间对照组(P<0.05),两组患儿SBP、TcSaO2水平较治疗前比较差异无统计学意义(P>0.05),且在治疗后2 h时比较,差异无统计学意义(P>0.05)。结论红细胞输注有助于改善早产儿脑氧代谢,改善患儿生命体征。

Abstract:【Objective】To explore the effects of red blood cell transfusion on cerebral oxygen metabolism and vital signs in premature infants.【Methods】A total of 160 cases of premature infants were selected as the research subjects.According to the random number table method,they were divided into study group and control group,with 80 cases in each group.The children patients in control group were given routine treatment,and the children patients in study group were combined with red blood cell transfusion on the basis of control group.The changes of cerebral oxygen metabolismcerebral tissue oxygen saturation(CrSO2),oxygen uptake fraction(CFOE)]were compared between the two groups before treatment and at 2 h after treatment.The vital signsheart rate(HR),systolic blood pressure(SBP),diastolic blood pressure(DBP),mean arterial blood pressure(MABP),percutaneous arterial oxygen saturation(TcSaO2)]were compared between the two groups before blood transfusion and at 2 h after treatment.【Results】At 2 h after treatment,the level of CrSO2 in study group was higher than that before treatment(P<0.05),and was significantly higher than that in control group at the same time point(P<0.05).The level of CFOE in study group was lower than that before treatment(P<0.05),and was significantly lower than that in control group at the same time point(P<0.05).At 2 h after treatment,the HR level in study group was lower than that before treatment(P<0.05),and was significantly lower than that in control group at the same time point(P<0.05),and the levels of DBP and MABP were higher than those before treatment(P<0.05),and were significantly higher than those in control group at the same time point(P<0.05).There were no statistically significant differences in the levels of SBP and TcSaO2 in the two groups compared with those before treatment(P>0.05),and there were no statistically significant differences between the two groups at 2 h after treatment(P>0.05).【Conclusion】Red blood cell transfusion can help improve cerebral oxygen metabolism and promote vital signs of premature infants.
