血清Hcy、CMKLR1水平在糖尿病视网膜病变患者中的表达及临床意义

摘    要:目的:探讨血清同型半胱氨酸(Hcy)、趋化因子样受体1(CMKLR1)水平在糖尿病视网膜病变(DR)患者中的表达及临床意义。方法:回顾性分析2020年6月至2021年6月该院收治的110例2型糖尿病(T2DM)患者的临床资料,根据是否合并DR分为DR组(n=72)和非DR组(n=38),其中DR组根据病情程度分为非增殖型组(n=41)和增殖型组(n=31),另选取40名同期体检的健康者作为对照组。所有受试者均检测血清Hcy、CMKLR1水平,比较DR组、非DR组、对照组Hcy、CMKLR1水平、非增殖型组与增殖型组Hcy、CMKLR1水平,以及Hcy、CMKLR1水平与DR的相关性及诊断效能。结果:DR组Hcy、CMKLR1水平均明显高于非DR组和对照组,且非DR组均高于对照组,差异有统计学意义(P<0.05);增殖型组Hcy、CMKLR1水平均明高于非增殖型组和对照组,且非增殖型组均高于对照组,差异有统计学意义(P<0.05);Hcy、CMKLR1水平与DR患者病情程度均呈正相关(P<0.05);受试者工作特征曲线分析显示,Hcy、CMKLR1诊断DR的曲线下面积分别为0.650、0.864;且联合检测诊断DR的灵敏度高于单项检测。结论:Hcy、CMKLR1水平与DR患者病情程度均呈正相关,二者联合检测诊断DR的效能高于单项检测。

Abstract:Objective:To explore efficacy of serum homocysteine(Hcy)and chemokine-like receptor 1(CMKLR1)levels in diagnosis of diabetic retinopathy(DR).Methods:100 patients with type 2 diabetes mellitus(T2DM)were selected as the research objects,and were divided into DR group(n=72)and non-DR group(n=38)according to whether they were combined with DR.The DR group was divided into non-proliferative group(n=41)and proliferative group(n=31)according to the severity of the disease.Another 40 healthy people who had physical examinations during the same period were selected as the control group.All these people were tested for the serum Hcy and CMKLR1 levels.The levels of Hcy and CMKLR1 in the DR group,the non-DR group and the control group were compared,and the serum Hcy and CMKLR1 levels in the non-proliferative group and the proliferative group were also compared.The correlations among the Hcy and CMKLR1 levels and DR and their diagnostic efficiencies were compared.Results:The serum Hcy and CMKLR1 levels in the DR group were significantly higher than those in the non-DR group and the control group;those of the non-DR group were higher than those in the control group;and the differences were statistically significant(P<0.05).The serum Hcy and CMKLR1 levels in the proliferative group were significantly higher than those of the non-proliferative group and the control group;those of the non-proliferative group was higher than those of the control group;and the differences were statistically significant(P<0.05).The levels of Hcy and CMKLR1 were positively correlated with the severity of the DR patients(P<0.05).ROC curve analysis showed that the levels of area under the curve(AUC)of Hcy and CMKLR1 in the diagnosis of DR were 0.650 and 0.864,respectively;and the sensitivity of combined detection was higher than that of single detection.Conclusions:The levels of Hcy and CMKLR1 are positively correlated with the severity of the DR patients.Moreover,the combined detection of the two levels is more effective in the diagnosis of DR than single detection.
