急诊手术治疗梗阻性结肠癌的体会

目的 探讨梗阻性结肠癌术式选择的原则。方法 回顾性总结112例梗阻性结肠癌急诊手术治疗结果。其中有半结肠切除术44例，左半结肠切除术31例，Hartmann手术10例。结肠次全切除术31例，低位前切除术7例，旁路手术6例，单纯结肠造瘘11例，结果 术后107例顺利恢复，手术效果满意，死亡2例（1.78%)，并发吻合口瘘3例（2.67%)。结论 梗阻性结肠癌，尤其是左侧结肠癌的术式选择应遵循个体化原则，有条件者主张一期切除吻合，免遭分期手术之苦，手术安全可行。

Experience of emergency operative treatment for obstructive colon cancer

Objective To investigate the principle of operative method-chosing for obstructive colon cancer. Methods The curative effect of emergency operation on 112 cases of obstructive colon cancer were analyzed. The article reported about 112 cases with obstructive colon cancer, 44 cases were subjected to resection of the right colon,31 to resection of the left colon, 10 to Hartmann operation, 3 to subtotal resection of colon, 7 to Dixon operation, 6 to bypass operation, 11 to simple colostomy. Results 107 cases were cured satisfyingly, 2 cases died, 3 cases were complicated with anastomose ostium fistula after operation. Conclusion It is emphasized that operative method should be according to individual condition of obstructive colon cancer patient, especially left colon cancer. If condition permit, primary resection and anastomosis are advocated. The patient will be prevented from suffering staged operations. This operation is safe and feasible.
