HRCT在活动性肺结核诊断中的价值

目的探讨活动性肺结核的HRCT表现及诊断价值。方法搜集经影像学检查、实验学检查及临床治疗综合诊断的活动性肺结核55例,回顾性分析其HRCT表现。结果本组55例中,活动性肺结核最常见的HRCT表现是：树芽样病灶（78.2%）,叶性实变（74.5%）,肺腺泡样结节（50.9%）,空洞（34.5%）和毛玻璃样病灶（32.7%）。边缘光滑、清晰的小叶中心性结节、支气管扩张、不规则线状索条和钙化是最常见的非活动性肺结核的CT表现。结论胸部X线检查是评价肺结核的主要和常用方法,HRCT在发现细小、隐匿病灶,鉴别可疑病灶,评价并发症和确定病灶是否活动方面有着不可替代的重要作用。

Value of High - resolution CT in the diagnosis of active pulmonary tuberculosis

Objective: To study the valuation and the appearance of high-resolution CT in the diagnosis of active pulmonary tuberculosis.Metheds: The HRCT findings were particularly reviewed in 55 cases confirmed by combined diagnosis by imaging examination,laboratory test and clinical treatment.Results: The most frequent HRCT patterns of active pulmonary tuberculosis were tree-in bud lesions(78.2%),lobular consolidation(74.5%),acinar nodule(50.9%),cavity(34.5%),and ground-glass opacity.(32.7%).The well-defined centrilobular nodule,bronchiectasis,irregular lines,and calcification were the most frequent HRCT findings in inactive pulmonary TB.Conclusion: Chest radiography is the mainstay in the evaluation of pulmonary TB,HRCT is generally useful to detect fine lesions overlooked on chest radiographs,to determine equivocal lesions,to evaluate complications or to determine the activity of pulmonary TB.
