脑胶质瘤切除程度及病理分级对生存率的影响

目的:探讨脑胶质瘤显微手术切除程度及肿瘤病理性质对患者长期生存的影响。方法:回顾69例实施显微手术治疗的脑胶质瘤患者临床资料,根据肿瘤切除程度、病理级别及随访结果分析患者的长期生存情况。结果:69例患者肿瘤全切除56例,次全切除10例,部分切除3例。术后随访1～5年,1年内死亡者4例,1～3年死亡15例,3～5年死亡21例熏存活5年以上者29例。其中肿瘤全切组1,3,5年生存率分别为89.3%,76.8%,50.0%;未全切组1,3,5年生存率分别为76.9%,53.8%,7.7%。按不同病理分级,低分级组(Ⅰ～Ⅱ级)的1,3,5年生存率分别为100.0%,87.7%,58.2%;高分级组(Ⅲ～Ⅳ级)1、3、5年生存率分别为80.0%,35.0%,2.5%。结论:肿瘤切除程度及病理性质是影响脑胶质瘤患者生存率的重要因素。

EFFECTS OF EXTENT OF RESECTION AND HISTOLOGICAL GRADE TO SURVIVAL RATE IN BRAIN GLIOMAS

Objective :To investigate the effects of resection extent under microsurgery and pathologic grade of tumor on long-term survival of the patients with brain gliomas.Method :Clinical data of69cases of operated patients with brain gliomas under microsurgery were analyzed retrospectively.The long-term survival data were got according to the resection extent,pathologic grade and follow-up survey.Res ults :Total,subtotal and part tumor resections were56,10and3cases respectively.The survival rates of1year,3years and5years were89.3% ,76.8%and50.0%respectively in total removal cases鸦those were76.9% ,53.8%and7.7%in non-total removal cases.According to pathologic grade of the tumor,the survival rates of1year,3years and5years were100.0%,87.7%and50.0%in grade Ⅰ ～ Ⅱ glioma鸦and80.0% ,35.0%and2.5%in Ⅲ ～ Ⅳ glioma respectively. Conclus ion :The results suggest that resection extent of the tumor and its histological characteristics are important prognostic factors for the survival rate of the patients with brain gliomas.
