严重烧伤病人早期肾功能损害与早期糖尿症的相关性研究

摘    要:目的:通过对严重烧伤病人进行应激性糖尿症和早期肾功能损害并发症观察研究,进一步了解其发病机制,方法:对1994年1月至2000年1月收住院的中-重度烧伤病人进行血Na,K,CI,Ca,Mg,P,BS,BUN,Cr,UA,TNFα、ISN、β2-MG及尿糖、尿PRO、尿pH,尿β2-MG等进行监测。结果发现5例病人在血糖不高的情况下出现糖尿和不同程度的肾功能损害,结论:部分严重烧伤病人早期糖尿是由肾脏近曲小管吸收障碍引起。

Abstract:Objective: To investigate the mechanism of stress glucosuria and renal function impairment in early stage of severe burn. Method: The levels of Na+ , K+ , Cl-, Ca+ + , Mg+ + , P, BS BUN, Cr UA, TNFα , INS, β -MG in blood and that of glucose, PRO, pH, β -MG in urine were monitored on patients with moderate and severe burns. Results: Five patients were found to have glucosuria and renal function impairment, while the blood sugar level was within normal range. Conclusion: Glucosuria in part of the patients may arise from re-absorption dysfunction of renal proximal convoluted tubules.
