雌激素治疗对围绝经期妇女抑郁相关症状的影响

背景雌激素降低更年期妇女骨质疏松发生率的作用已被证实,雌激素治疗是否具有改善围绝经期妇女抑郁相关症状的作用?目的观察雌激素对围绝经期妇女抑郁相关症状的影响设计病例对照研究.地点和对象在山东省立医院院妇产科门诊就诊的围绝经期患者,选择60例主诉有抑郁症状者,要求神志清楚、检查合作、知情同意并按要求用药及完成测评,年龄45～55岁.干预将60例围绝经期抑郁患者随机分成两组,每组30例.实验组给予戊酸雌二醇(广州先灵药业有限公司生产,批号588B2-4)1mg,1次/d;谷维素(济宁安康制药厂生产,批号010418)10mg,3次/d;对照组给予谷维素10mg,3次/d.两组治疗时间均为21 d.于治疗前后分别应用汉密顿抑郁量表(HAMD)评定患者抑郁程度.主要观察指标两组患者服药前后抑郁改善程度.结果实验组HAMD减分率[(9.8±7.7)%]明显高于对照组[(15.3±5.4)%],差异有非常显著性意义(t=25.924,P＜0.01);实验组抗抑郁效果明显优于对照组[组显效为26例,对照组显效为3例],差异有显著性意义(x2=35.312,P＜0.01).结论雌激素可改善围绝经期抑郁症患者的情绪和行为,降低抑郁症分值,改善抑郁状态.

Effect of estrogen onsymptoms related with depression in peri-menopausal women

BACKGROUND: It has been proved that estrogen decreases the occurrence of osteoporosis in menopausal women. Does estrin treatment have the function of improving the symptoms related with depression in peri-menopausal women?OBJECTIVE: To observe the effect of estrogen on the symptoms related with depression in peri-menopausal women.DESIGN: A case-controlled study.SETTING and PARTICIPANTS: Totally 60 peri-menopausal patients complaining of symptoms of depression in the Out-patient Clinic, Department of Obstetrics & Gynecology of Shandong Provincial Hospital were selected. They were required to be sane, cooperative and consented to administration of medicines and evaluation according to the rules and with the age of (45 -55) years.INTERVENTIONS: The 60 depressive patients in the peri-menopausal period were randomly divided into two groups, 30 in each. The experiment group received 1 mg progynova (Guangzhou Xianling Pharmacy Co. Ltd., qualification No. 588B2-4)and 10 mg gammariza(Jining Ankang Pharmacy Co. Ltd, Qualification No. 010418) three time per day; The control group received 10 mg gammariza three times per day. The course was 21 days. The depressive degree was assessed by Hamilton Rating Scales for Depression(HAMD) before and after treatment.MAIN OUTCOME MEASURES: The improvement of depression of patients in the two groups before and after administration.RESULTS: Decreased rate of HAMD in the experimental group [(59.8±7.7)%] was significantly higher than that of the control group [(15.3±5.4)%], and the differences had significant meaning(t = 25. 924, P ＜ 0.01); Effect of anti depression was significantly better than that in the control group(26 cases showed effect in the experimental group, 3in the control group), and the difference had significant meaning(x2= 35. 312, P＜0.01).CONCLUSION: Eestrogen can improve the emotion and behavior of depressive peri-menopausal patievts,dedrease depression value and ameliorate depressie status.
