脓毒症大鼠脾细胞凋亡、凋亡相关蛋白表达及中药血必净的影响

摘    要:目的观察血必净对脓毒症大鼠脾脏淋巴细胞凋亡、凋亡相关蛋白表达以及半胱天冬酶(caspase)3活性的影响。方法96只Wistar大鼠随机分为正常对照组(8只)、假手术组(8只)、模型组(40只)和血必净治疗组(40只),以盲肠结扎穿孔法(CLP)制备脓毒症模型;TUNEL法检测脾细胞凋亡情况、分光光度法测定脾组织半胱天冬酶3活性、流式细胞仪检测脾脏淋巴细胞中CD3+细胞比例以及免疫组化法检测脾脏FasL、Bcl-2蛋白表达水平。结果假手术组大鼠脾脏FasL表达的IOD值为4.54±1.41,模型组较之明显增多,至48 h达757.96±188.10,假手术组大鼠脾脏Bcl-2表达的IOD值为594.05±183.32,模型组表达下降,于48 h仅为22.71±9.22,假手术组仅有少量细胞凋亡(0.87±0.51),半胱天冬酶3活性较低(4.34±1.34),CD3+细胞比例为63.49%;模型组于造模后4 h起脾组织细胞凋亡数增多达8.88±3.68,半胱天冬酶3活性升高至12.22±3.15,而脾细胞中CD3+细胞比例明显下降为52.09%;血必净干预后可明显降低脾脏FasL表达、促进Bcl-2表达,减轻脾组织半胱天冬酶3活化程度,减少脾细胞凋亡,并使CD3+细胞比例恢复正常。结论脓毒症时凋亡途径的活化使脾细胞过度凋亡,引起了免疫失衡的发生,血必净可以通过调节凋亡相关蛋白的表达,减轻脾细胞凋亡,恢复脓毒症的免疫平衡。
