| 摘 要： | 目的探讨显微技术小骨窗开颅行丘脑血肿清除术＋脑室外引流术与丘脑血肿穿刺引流术＋脑室外引流术治疗脑室型丘脑出血的疗效。方法46例脑室型丘脑出血（大于20ml）患者随机分成A、B两组，A组22例，采用显微技术小骨窗开颅丘脑血肿清除术＋脑室外引流术；B组24例，在CT定位下行丘脑血肿穿刺引流术＋脑室外引流术。结果A组治疗在意识恢复时间、死亡率、GOS评分与B组比较差异有统计学意义（P〈0．05）。结论丘脑血肿大于20ml的脑室型丘脑出血宜采用显微外科小骨窗开颅行丘脑血肿清除术＋脑室外引流术。
