中心静脉导管闭式引流在胸腔积液中的应用

目的探讨中心静脉导管闭式引流在胸腔积液治疗中的应用价值。方法以210例不同病因的胸腔积液患者为研究对象，所有患者均应用中心静脉导管行胸腔闭式引流。结果所有患者均一次置管成功，4例出现气胸经引流管抽气后治愈；10例导管阻塞用导丝或生理盐水冲管后复通；2例导管脱落重新置管。所有患者经4～14d置管胸液引流彻底。恶性胸液患者经导管胸腔内注入药物。结论中心静脉导管胸腔闭式引流与常规胸穿放液相比是一种安全、可靠损伤小的方法，具有可行性、安全性，值得临床推广应用。

Application of Central Venous Catheter Closed Drainage to Pleural Effusions

Objective To explore the application of central venous catheter closed drainage to patient with pleural effusions. Methods Central venous catheter closed drainage were applied to 210 patients with pleural effusions with various causes. Results All the patients were successfully intubated for the first time. Pneumothorax that happened in 4 patients was cured sucessfully through the central venous catheter. Catheters were blocked in 10 patients which recanalized with guidewire or sodium chloride injection. Catheter fell off in 2 patients. Pleural effusions of al 1 the patients disappeared thoroughly during 4 - 14 days＇s drainage. By the way, the malignant pleural effusion patients were given drugs through the catheter. Conclusion Central venous catheter closed drainage is safer than routine thoracentesis. It possesses feasibility, safety and advantage, and deserves widely clinical application.
