移位髋臼骨折的手术治疗

目的探讨移位髋臼骨折（DAF）的手术方法和疗效。方法选择DAF54例，内固定手术采用髂腹股沟人路9例，Kocher-Langenbeck人路29例，联合人路16例。结果解剖复位44例（81％），满意复位8例（15％），不满意复位2例（4％）。随访1～6年。临床和X线优良率分别为81％和83％，其中解剖和非解剖复位组的临床优良率分别为88％和52％（P〈0．01）。结论（1）手术治疗DAF可获得满意的骨折复位和临床疗效；（2）满意的骨折显露，并遵循合理的复位固定程序，是改善骨折复位质量、提高疗效的关键。

Surgical treatment of displaced acetabular fractures

Objective To summarize the surgical techniques and results in the treatment of displaced acetabular fractures(DAF).Methods Fifty-four cases of DAF were treated with open(reduction) and internal fixation.Ilio-inguinal approach was adopted in 9 cases,Kocher-langenbeck approach in 29 cases,and double approachs(K-L ilio-inguinal) in 16cases.Results 44 cases(81%) had anatomic reduction,with satisfactory reduction in 8 cases(15%),unsatisfactory in 2 cases(4%).The results of 1-6 year follow-up were excellent and good rate for clinical and roentgenographic in 81% and 83% respectively,which were 88% and 52% in anatomic and non-anatomic groups respectively.Conclusion(1) Operative treatment is able to have satisfactory fracture reduction and clinical(results.)(2)To get satisfactory exposure and to observe an ideal reduction and internal fixation process are the key for satisfactory reduction and good clinical results.
