慢性阻塞性肺病急性加重期心肌酶谱的变化

摘    要:目的:探讨慢性阻塞性肺疾病(COPD)患者急性加重期低氧血症对心肌酶水平的影响。方法:68例COPD急性加重期患者按动脉血氧分压的水平分成3组。测定各组患者动脉血氧分压(PaO2)及血清天冬氨酸转氨酶(AST)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)、α-羟丁酸脱氢酶(HBDH),并分析其PaO2与血清心肌酶水平的相关性。结果:重度低氧血症组患者血清心肌酶水平明显高于中度低氧血症组和轻度低氧血症组(P<0.01)。COPD急性加重期患者PaO2水平与对应的血清心肌酶呈直线负相关(r=-0.67~-0.87,P<0.01)。结论:低氧血症可导致COPD患者血清心肌酶水平增高,且动脉血氧分压与相应的心肌酶呈直线负相关。

Abstract:Objective: To investigate the effects of hypoxemia on serum myocardial enzymes in patients with chronic obstructive pulmonary disease (COPD) during acute exacerbation period.Methods:The 68 COPD patients during acute exacerbation period were divided into three groups according partial pressure of oxygen in artery (PaO2).Serum aspartate aminotransferase (AST), creatine kinase (CK), creatine kinase isoenzyme (CK-MB), lactate dehydrogenase (LDH), hydroxybutyrate dehydrogenase (HBDH) and PaO2 were measured in patients of three groups. The changes of serum myocardial enzymes and PaO2 , and its relations were observed in three groups. Results:The levels of above serum enzyme of severe hypoxemia group increased than those of moderate and mild hypoxemia groups (P<0.01).The level of PaO2 had a negative relation with serum myocardial enzymes(r = -0.67~-0.87,P<0.01) in the COPD patients during acute exacerbation period.Conclusion:Hypoxemia may significantly lead to increase of serum myocardial enzymes in COPD patients, and PaO2 level has a significant negative relation with myocardial enzymes.
