基于生物学指标的糖尿病肾病血瘀证研究

摘    要:目的:探索糖尿病肾病血瘀证与微观指标之间的关系,为糖尿病肾病血瘀证提供客观化依据。方法:多中心收集糖尿病肾病182例,辨证为血瘀证104例,非血瘀证78例,以30例健康人为正常对照,检测ET、NO、t-PA、PAI-1、HbA1c、血脂及血液流变学指标,用统计学方法分析上述指标与各组之间的关系。结果:(1)与正常组比较,糖尿病肾病组所有上述观察指标差异均有统计学意义(P<0.05,P<0.01);(2)与非血瘀证组比较,血瘀证组ET、NO、t-PA、PAI-1、HbA1c、TG、HDL、LDL、Fib、PV、TK及LSR差异有统计学意义(P<0.05,P<0.01);TC、HSR差异无统计学意义(P>0.05)。结论:糖尿病肾病血瘀证患者存在血糖、血脂、血液流变学、纤溶及血管内皮系统异常,可为糖尿病肾病血瘀证客观化提供依据。

Study of Blood Stasis Syndrome in Diabetic Nephropathy Based on Microscopic Indexes

Abstract:To explore the relationship between the levels of microscopic indexes and the syndrome of blood stasis in the patients with Diabetic Nephropathy(DN) and to provide objective evidence for it. Methods:IS2 patients multicenrtic with DN were divided into two groups: syndrome of blood stasis(group of A,n = 104) ,and syndrome of non - blood stasis (group of B,n = 78). the normal ( group of C, n = 30) were took as the control. The indexes in serum or plasma were detected, which were nitric oxide (NO) .endothelin(ET) .tissue type plasminogen activator ( t - PA) .human plasminogen activator inhibitor 1 (PAI - 1 ) ,HbAlc.Tri glyceride ( TG). total cholesterol ( TC ). low density lipoprotein ( LDL), high density lipoproteins ( HDL ). fibrinogen ( Fib ). plasma vis- cosity (PV) .lower shear rate ( LSR ), higher shear rate (HSR) and erythrocyte metamorphosis index ( TK ), the relationship between the syndromes and the levels of microscopic indexes were analyzed by the statistical method. Results: ( 1 ) There was statistical significance in all the indexes above between group of C and group of DN ( P 〈 0. 05, P 〈 0.01 ) ; (2) There was statistical significance in indexes of ET. NO. t - PA. PAI- 1. HbA 1 c. TG. HDL, LDL. Fib. PV, TK and LSR between group of A and group of B ( P 〈 0. 05, P 〈 O. O1 ), while there was not statistical significance in TC. I-ISR between them ( P 〉 0.05 ). Conclusion: Abnormality developed in blood glu- cose.blood lipid.hemorhoology.vasoactive factor and also in fibrinolytic activity of patients with syndrome of blood stasis in DN.
