有创-无创序贯机械通气治疗急性重度有机磷中毒合并中间综合征疗效观察

摘    要:目的评价有创-无创序贯机械通气对急性重度有机磷中毒合并中间综合征的治疗价值。方法选择100例患者,随机分为有创-无创序贯机械通气治疗组和有创机械通气(IPPV)组作为对照组,各50例。动态观察两组患者呼吸机相关肺炎(VAP)发生率、有创机械通气时间、总机械通气时间、住院时间、再插管率等情况。结果有创-无创序贯机械通气治疗急性重度有机磷中毒合并中间综合征,呼吸机相关性肺炎(VAP)发生率、有创机械通气时间、住院时间及再插管率明显低于对照组(P〈0.05)。结论有创-无创序贯机械通气治疗急性重度有机磷中毒合并中间综合征可降低呼吸机相关性肺炎(VAP)发生率、有创机械通气时间、住院时间及再插管率。

Analysis to the sequential mechanical ventilation in acute organophosphate poisoning with intermediate syndrome

Abstract:To evaluate the possibility and the clinical value of the sequential mechanical ventilation in acute organophosphate pesticides poisoning(AOPP)with intermediate syndrome.Methods One hundred patients with intermediate syndrome were included in the study.Of them,50 were randomly allocated to receive sequential mechanical ventilation,and the other 50 received invasive positive pressure ventilation(IPPV).And ventilator-associated pneumonia(VAP),mechanical ventilation duration,weaning failure,the length of hospitalization was measured.Results There were significant differences in invasive ventilation time,the total duration of mechanical ventilation,venti1ator-associated pneumonia(VAP),weaning failure,the length of hospitalization and mortality between the sequential treatment group and the control group(P〈0.05).Condusion Invasive-noninvasive ventilation is effective in the treatment of AOPP cases with intermediate syndrome,it can significantly shorten the time of invasive ventilation,the length of hospitalization and reduce the incidence of VAP,weaning failure.
