单纯应用Cage及椎弓根钉技术与加用椎间自体植骨治疗腰椎滑脱的疗效比较

目的讨论后路椎间融合(PLIF)应用Cage及椎弓根钉技术(A组)后有无必要加用椎间自体植骨(B组)治疗腰椎滑脱。方法回顾性研究27例资料,随访时间最少不短于24个月,对两组患者进行失血量、住院日、疼痛度及缓解程度、融合率及椎间隙高度、术后滑脱率及融合率进行分析比较。结果两组患者在失血量、住院时间、疼痛度及缓解程度、融合率均无明显差异(P>0.05),但B组在术后椎间隙高度、滑脱率上均存在明显优势(P<0.05)。结论PLIF手术应用椎间植自体骨,Cage及椎弓根钉技术有助于恢复和保持腰骶椎生理曲度,防止后期的椎间隙高度丢失及滑脱率的增加,可能有利于长期手术疗效的保持。

Comparison between Autologous Bone Grafting and Cage Fusion with Pedicle Screw Fixation and Simple Cage Fusion with Pedicle Screw Fixation in Posterior Lumbar Interbody Fusion for Treatment of Adult Spondyiolisthesis

Objective To compare the clinical outcomes of PLIF using autologous bone grafting and cage fusion with pedicle screw fixation(group 2) and simple cage fusion with pedicle screw fixation (group 1)in adult spondylolisthesis.Methods Twenty-seven patients with minimum follow-up of 24 months,treated by interbody fusion with pedicle screw fixation combined cage were retrospectively studied.Two groups were compared for the height of the intervertebral disc space,the postoperative degree of slip and the fusion rate.Results There was no significant difference between the two groups in the amount of blood loss,the duration of hospital stay,the back pain,the radiating pain (P>0.05).Group 2 had significant advantages over group 1 in the height of the disc space, the slip rate and the fusion time (P<0.05).Conclusion The PLIF using autologous bone grafting and the cage fusion with pedicle screw fixation is more favorable for the fusion rate and the long-term stability than the simple cage fusion with pedicle screw fixation in adult spondylolisthesis.
