摘    要:目的]探讨B超监护下宫腔镜对子宫切口妊娠物的诊治价值.方法]对28例子宫切口妊娠物的患者在B超监护下经宫腔镜进行诊断、定位后吸取或镜下直接钳取残留胚物等治疗.结果]28例中27例成功,其中1例胚胎吸出后出血较多,经纱条填塞等止血处理后成功.B超监护宫腔镜下清宫一次成功率96.4%.1例经宫腔镜定位吸出大块胚物后,B超提示子宫切口穿透,立即开腹手术,术中见病灶组织已侵蚀膀胱,行病灶清除并修补切口.结论]B超监护下宫腔镜具有安全、直接、准确、微创的特点,对子宫切口妊娠物的诊治是极其有效的方法.

Abstract:Objective]To investigate the diagnostic and therapeutic value of hysteroscopy under B-ultrasound monitoring in the treatment of pregnancy tissue after uterinel incision. Methods] Twenty-eight cases of pregnancy tissue after uterine incision were diagnosed , located and taked out by forceps under hysteroscopy with B-ultrasound monitoring. Results] Twenty-seven cases were successful; there was one case who had massive hemorrhage afer taking out the residual pregnancy tissue and cured by gauze strip packing method. Another case was taken out big block of residual tissue and the rupture of uterus was observed and treated by laparotomy. Eroded bladder lesion was seen in the operation; clearance of focal lesion and repair were performed. The successful rate at one time was 96.4%. Conclusion] Hysteroscopy under B-ultrasound monitoring is safe,direct,accurate and minimally invasive for the diagnosis and treatment of pregnancy tissue after uterine incision.
