非心源性胸痛65例临床分析

目的 探讨非心源性胸痛（NCCP）的临床特征及诊断要点.方法 对65例NCCP患者的原始疾病、临床表现特征及诊治情况等临床资料进行分析.结果 65例NCCP患者中消化系统疾病27例（41.5％）,呼吸系统疾病21例（32.3％）,血管疾病10例（15.3％）,胸壁胸膜疾病5例（7.6％）,其他类疾病2例（3.0％）.结论 NCCP病因复杂,症状、体征、理化检查往往与心源性胸痛有相似性,诊断和鉴别诊断存在一定难度,主动脉夹层、肺栓塞等NCCP病情凶险,临床实践中尤应详加鉴别.

Clinical analysis of 65 cases with non-cardiac chest pain

Objective To discuss clinical characteristics and diagnosis of non-cardiac chest pain （NCCP）. Methods The study collected clinical data from a cohort of 65 patients with NCCP. Results In the 65 patients, the pain was diag- nosed to be gastrointestinal in 27（42.5%）, respiratory in 21 （32.3%）, vascular in 10 （15.3%）, thoracic in 5（7.6%） and miscellaneous in 2（3%）. Conclusion NCCP has a wide range of potential causes. It may produce symptoms, signs and results of laboratory tests similar to angina. A patient＇s history and characteristics do not reliably distinguish be- tween cardiac and non-cardiac chest pain. Although NCCP is in general associated with a good prognosis, some disor- ders, such as pulmonary embolus, pneumothorax and aorticdissection, are severe and life-threatening conditions. There- fore diagnosing the causes of NCCP becomes important and necessary.
