摘    要:目的探讨经支气管镜肺活检对周围型肺癌的诊断价值.方法通过胸部正侧位片及胸部CT片确定病灶部位后,对常规经支气管镜检查无法观察到病变的疑似周围型肺癌患者66例行经支气管镜肺活检.结果经支气管镜活检确诊肺癌28例,刷检细胞学检查确诊2例,二者合并诊断阳性率为45.4%.检查过程中出现气胸1例,咯血3例,无1例死亡.结论经支气管镜对周围型肺癌肺活检方法简单易行,并发症少,能显著提高周围型肺癌的诊断阳性率,值得推广应用.

Abstract:Objective To investigate the diagnostic value of transbronchoscopic lung biopsy for peripheral lung cancer.Methods Sixty-six patients, which were doubt as peripheral lung cancer and could not be viewed by normal bronchoscopy, were transbronchoscopic lung biopsy after determined the focus position by chest X-ray photo and computerized tomography (CT).Results Twenty-eight cases were diagnosed lung cancer by transbronchoscopic lung biopsy and 2 cases were diagnosed lung cancer by brunshing cytological examination. The whole diagnosis positive rate was 45.4%. Pneumothorax was occurred in 1 case and hemoptysis were occurred in 3 cases among examination. No one was dead in examination.Conclusion The technique of transbronchoscopic lung biopsy has obvious advantages and was worth to application in clinical diagnosis for peripheral lung cancer due to its convenience, low incidents of serious side effects and high diagnosis rate.
