单开门治疗多节段颈椎病颈髓损伤

目的：探讨单开门椎板扩大成形术治疗多节段脊髓型颈椎病颈髓损伤的疗效及其相关影响因素。方法采用改良单开门手术，C3-C6椎板单开门扩大成形治疗21例脊髓型颈椎病患者。结果随访7-36个月；平均21个月。JOA评分由术前的（7.1±2.3）分提高到随访时的（13.3±2.7）分；其中优8例，良9例，可4例，差0例。测量骨性椎管矢状径术后平均增加5.0mm。13例患者在术后6-18个月复查颈椎MRI提示术前颈髓受压高信号明显恢复。结论改良单开门椎板成形术是一种安全和有效的术式，椎管骨性面积的增加是神经功能改善的基础。

Modified single-door laminoplasty for the treatment of cervical spondylotic myelopathy

Objective To investigate the outcomes and the factors affecting the prognosis by modified single-door laminoplasty in the cervical spondylotic myelopathy（CSM）. Methods 17 patients with cervical CSM underwent modified single-door laminoplas-ty from C3 to C6. The period of follow-up lasted from 7 to 36 months with a mean of 21 months. Neurologic deficits pre-and post-operation were assessed using a scoring system proposed by the Japanese Orthopaedic Association （JOA score）. Results The JOA scores were improved from points pre-operation to points at the latest follow-up. Of all patients,6 cases were excellent,7 cases were good,4 cases were fair and no case was poor. There was significant enlargement of sagittal diameter of cervical canal by 5.0mm after surgery. In 11cases,whose spinal cord compression recover well showed in magnetic resonance imaging （MRI） images after operation 6-18months. Conclusion Modified expansive single-door laminoplasty is a reliable procedure for CSM.Improve-ment of neurologic function is based on enlargement of cervical canal.
