肥胖对老年高血压左室肥厚和左心功能的影响

目的：探讨肥胖对老年高血压左室肥厚和左心功能的影响。方法：应用超声心动图检测50例老年高血压伴肥胖者左室重量、左室重量指数和左心功能，并取53例老年高血压非肥胖者作为对照。结果：肥胖组左室重量和左室重量指数均明显高于非肥胖组（P〈0．01）；肥胖组左室肥厚检出率也较非肥胖组明显增高（P〈0．01），前者发生左室肥厚的相对危险度是后者的2．12倍。肥胖组峰值速度E／峰值速度A比值均明显下降（P〈0．01），而峰值速度A和左室射血分数变化不明显（P〉0．05）。结论：肥胖对老年高血压左室肥厚有促进作用，可引起舒张功能障碍。

Influence of obesity on left ventricular hypertrophy and left ventricular function in the aged patients with hypertension

Objective To study the influence of obesity on left ventricular hypertrophy and left ventricular function in the aged patients with hypertension. Methods One hundred and three patients with hypertension were divide into obesity and non-obesity group. The left ventricular mass, left ventricular mass index and left ventricular function in the aged patient were measured with echocardiography technique. Results Left ventricular hypertrophy and left ventricular mass index were higher significantly in the aged patients with obesity than non-obesity group(P<0.01). The rates of left ventricular hypertrophy in the aged patients of hypertension with obesity were obviously improved compared with those of patients without obesity (P<0.01). The relative risk of left ventricular hypertrophy in patients with obesity group was 2.12 times higher compared with those without obesity. E and E/A indexes were all decreased in the patients with obesity group than those of patients without obesity (P<0.01), but A and LVEF was not significantly different between patients with obesity group and patients without obesity (P>0.05). Conclusion Obesity can aggravate left ventricular hypertrophy and decrease left ventricular diastolic function in the aged patients with hypertension.
