德湿康填塞时间对鼻显微手术疗效的影响

目的:分析德湿康藻酸钙敷料填塞时间对鼻显微手术疗效的影响。方法:回顾分析2005年1月至4月在我院诊为双侧鼻息肉、慢性鼻窦炎,且双侧病变程度相似的44例的临床资料,其中男31例,女13例,20~85岁,平均49.6岁。患者均行双侧鼻显微手术,随机将双侧鼻腔分为两组,试验组侧鼻腔填塞浸润庆大霉素16万U和地塞米松5mg的德湿康藻酸钙敷料,术后7d取出填塞物或待其自行脱落;对照组侧填塞干德湿康藻酸钙敷料,术后48h取出填塞物。观察鼻出血、疼痛度、鼻腔黏膜水肿程度、分泌物渗出,测量鼻通气阻力和鼻声反射,每日观察并清理鼻腔内干痂,记录取干痂的持续时间和疼痛度。结果:两组止血效果均满意,无1例需加强填塞。抽取填塞物时试验组1例有少量渗血,对照组6例有少量渗血,3例因出血较多需重新填塞(P<0.05);其余指标试验组均好于对照组,统计学显示差异有高度显著性(P<0.01)。结论:延长德湿康藻酸钙敷料的填塞时间能有效地降低取出填塞物时的再出血率和疼痛度,且术后创面结痂少,愈合好。

Packing sorbalgon tamponade strip in microscopic sinus surgery : effect of time

Objective: To study the effect of sorbalgon tamponade strip packing duration on the microscopic sinus surgery. Methods: The retrospectively analysis on the duration of sorbalgon tamponade strip packing was carried out in a sample of 31 males and 13 females,who were diagnosed as double side nasal polyps and chronic sinusetis with homoplastic degree.The left and right sides were randomly divided into experimental one packed in by sorbalgon tamponade strips infiltrated gentamycin and dexamethasone which was taken out 7 days later,and control one packed in by dry sorbalgon tamponade strips taken out 2 days later.Bleeding,pain,mucosal edema in nasal cavity,secretion effusion degree,nasal airway resistance and nasal acoustic reflection were observed and measured;the duration of the scab form in the nasal cavity was recorded. Results:Capillary hemorrhage happened in 1 case in the experimental group,in 6 cases in the control group when being taken out the wadding and 3 out of them needed re-packing(P<0.05).Other indexes improved significantly in the experimental group(P<0.01). Conclusion: The results of examined indexes are significantly promoted in the experimental group,suggesting that there is a low rate of haemorrhagia,pain and scab by lengthening the packing time of sorbalgon tamponade strips infiltrated gentamycin and dexamethasone.
