保胆取石术治疗胆囊结石的临床应用

目的：采用纤维胆道镜行保胆取石手术治疗胆囊结石，与小切口胆囊切除术和开放式胆囊切除术比较，探讨选择性保胆取石术治疗胆囊结石的可行性。方法：保胆取石组66例，小切口胆囊切除组146例，开放式胆囊切除组92例，分别观察患者的手术时间、住院天数、术后切口感染、饮食恢复时间和体重变化的差异。结果：保胆取石组61例患者术后随访3—6个月，症状消失或减轻者占8l％，B超复查胆囊均未发现结石。手术时间及住院天数缩短，术后切口感染、饮食恢复及体重变化方面均优于小切口胆囊切除组和开放式胆囊切除组。结论：保胆取石术具有创伤小、安全性高、并发症少等优点，是胆囊结石患者手术治疗的合理方法之一。

The application of cholecystolithotomy with gallbladder preserved in surgicaltreatment of gallstone disease

Objective: To explore the effect of cholangioscopic cholecystolithotomy with gallbladder preserved in surgical treatment of gallstone diseases. Methods:66 patients with gallstone were selected to undergo cholangioscopic cholecystolithotomy with gallbladder preserved from Oct 2000 to Oct 2002. While 146 patients received minicholecystectomy and another 92 patients received classic cholecystectomy. Results: All patients undergo cholangioscopic cholecystolithotomy obtained complete recovery without relapse during follow-up. No related complications were observed. Conclusion:Compared with other surgical methods, cholangioscopic cholecystolithotomy with gallbladder preserved is an effective, micro-traumatic and safe method in treating gallstone diseases while preserving the gallbladder function.
