载脂蛋白CIII与冠心病及血脂的相关性以及他汀类药物对其的影响

目的探讨血浆载脂蛋白CIII(Apo CIII)与冠心病(CHD)及传统血脂指标的关系以及他汀治疗对其的影响。方法选取2012年4月~2013年3月在阜外医院就诊的患者143例,按照CHD诊断标准分为非冠心病(non-CHD)组80例和CHD组63例。CHD组患者均接受他汀药物治疗并接受随访。记录所有患者临床资料并采集空腹静脉血,检测患者血浆中传统血脂指标并利用ELISA试剂盒检测患者血浆Apo CIII水平。统计分析non-CHD组与CHD组以及CHD他汀类药物治疗后血浆Apo CIII水平的差异,并分析Apo CIII与传统血脂指标的相关性。结果 CHD组患者血浆Apo CIII水平低于non-CHD组,他汀治疗能显著升高CHD患者血浆中Apo CIII的水平(P0.05)。Apo CIII与三酰甘油在non-CHD组患者显著相关(P0.05)。在CHD组患者中,Apo CIII与血浆中三酰甘油(r=0.464,P0.05)和总胆固醇(r=0.307,P0.05)水平显著相关,并不受他汀治疗的影响。CHD患者血浆中Apo CIII与低密度脂蛋白胆固醇(LDL-c)的相关性仅在他汀治疗后具有显著的相关性(r=0.459,P0.05)。结论血浆Apo CIII水平与CHD和传统血脂指标具有一定的相关性,并在一定程度上受他汀治疗的影响。

The associations of plasma Apo CIII with coronary heart disease and lipids and the effect of statin treatment on them

Objective To study the associations of Apo CIII with coronary heart disease (CHD) and plasma lipids and the effect of statin treatment on them. Methods A total of 143 patients were enrolled,included 80 non-CHD patients and 63 CHD patients,and CHD patients received statin treatment and were followed up. Clinical information of all the patients was recorded,and fasting vein blood was collected to measure lipids and Apo CIII level. The associations of Apo CIII with CHD and lipids and the effect of statin treatment on them were analyzed by statistic method. Results The Apo CIII level of CHD was lower than non-CHD,and it was elevated significantly after statin treatment (P<0.05). The association of Apo CIII with triglyceride was significant in non-CHD (P<0.05). The associations of Apo CIII with total cholesterol and triglyceride were significant in CHD patients (P<0.05),and the associations between them were not affected by statin. The association of Apo CIII with low density lipoprotein cholesterol was significant in CHD patients after statin treatment only. Conclusion There are some associations of Apo CIII with CHD and lipids parameters,and it is also affected by statin treatment in some extent.
