以家庭为中心的健康教育在提高甲状腺癌术后患者甲状腺素服药依从性中的应用

目的 探讨以家庭为中心的健康教育模式对甲状腺癌术后患者甲状腺素服药依从性的影响.方法 便利抽样法选取2015年2-12月于天津医科大学肿瘤医院住院手术治疗的甲状腺癌患者206例,按入院先后将其分为观察组106例和对照组100例,出院后两组患者均按医嘱口服左甲状腺素钠片,观察组实施以家庭为中心的健康教育模式,对照组按常规进行健康教育.于患者出院后1、3个月门诊复查时采用Morisky用药依从性问卷(Morisky medication adherence scale,MMAS-8)对患者的服药依从性进行评定,并收集患者复查时的血清促甲状腺激素(thyrotropin,thyroid stimulating hormone,TSH)结果.结果 出院后1、3个月,观察组患者的服药依从性高于对照组,差异均有统计学意义(均P＜0.05);观察组患者TSH水平维持在推荐水平的人数多于对照组,差异有统计学意义(P＜0.05).结论 以家庭为中心的健康教育可提高甲状腺癌患者术后服药依从性,维持血清TSH处于稳定水平.

Application of Family-centered Health Education in Improving the Thyroxine Medication Compliance of Postoperative Patients with Thyroid Cancer

Objective To discuss the effect of family-centered health education in improving the thyroxine medication compliance of postoperative patients with thyroid cancer.Method By using convenience sampling,206 cases were selected and divided into observation group(n=106) and control group(n =100).Patients in two groups both had Levothyroxine.The observation group received family-centered health education mode,while the control group received routine health education.The Morisky medication adherence scale (MMAS-8) was used to evaluate the medication compliance after discharged one and three months.The thyrotropin,thyroid stimulating hormone (TSH) result was collected.Results After discharged from hospital for one and three months,the medication compliance in observation group was higher than those in control group (all P＜0.05).The number of patients' TSH level maintained in recommendation level in observation group was higher than those of control group (P ＜ 0.05).Conclusions The family-centered health education can improve the medication compliance of postoperative patients with thyroid cancer,and also contributes in maintaining TSH level.
