经肛管内外括约肌间切除术在超低位直肠癌保肛术中的应用

目的探究经肛管内外括约肌间切除术在超低位直肠癌保肛术中的应用效果。方法收集本院的超低位直肠癌患者47例,采用随机数字分配法分为治疗组与对照组,其中治疗组24例,实施经肛管内外括约肌间切除术;对照组23例,实施经腹直肠癌前切除术。治疗结束后评价术后3个月、6个月以及12个月肛门功能满意率及肛门失禁评分情况。结果治疗组与对照组相比,术后并发症发生率较低,差异有统计学意义(P0.05);肛门功能满意率上升,差异有统计学意义(P0.05);肛门失禁评分呈下降趋势,差异有统计学意义(P0.05)。结论经肛管内外括约肌间切除术在超低位直肠癌保肛术中的应用效果佳,可以提高患者的生存质量,是治疗低位结肠癌的有效手段,适宜临床应用推广。

Effect of the ultra low rectal cancer anus preserving operation by anal intersphincteric resection

Objective To investigate the effect of the ultra low rectal cancer anus preserving operation by anal intersphincteric resection. Methods 47 ultra low rectal cancer from our hospital were selected and randomly divided into the control group and the experiment group. 24 cases in the experiment group were treated by anal intersphincteric resection and 23casses in the control group were treated by anterior resection of rectal carcinoma. The anal function satisfaction rate and anal incontinence score were compared in 3 months,6 months and 12 months after treatment. Results Compared with the control group,the postoperative complication rate were lower (P<0.05),the anal function satisfaction rate were higher (P<0.05),the anal incontinence score were lower(P<0.05). Conclusion The internal and external anal sphincter resection operation in super low rectal cancer preserving anus saving operation has good clinical effect,improves the quality of life of patients,and is suitable for clinical application.
