异基因造血干细胞移植患者的生活质量现况及其影响因素

目的探讨成人异基因造血干细胞移植(hematopoietic stem cell transplantation,HSCT)患者移植后生活质量状况及其影响因素,为提高其生活质量提供依据。方法 2016年6-11月,便利抽样法选择苏州大学附属第一医院129例异基因造血干细胞移植术后患者为研究对象,采用患者一般资料调查表以及癌症治疗功能评价系统-骨髓移植生活质量测评量表(functional assessment of cancer therapy-bone marrow transplantation scale,FACT-BMT)进行问卷调查。结果患者的FACT-BMT生活质量总分为(107.40±20.22)分,生理健康状况得分为(21.91±5.66)分,社交/家庭健全情况得分为(20.78±5.00)分,情绪稳定状况得分为(19.65±3.96)分,功能健全状况得分为(15.15±7.07)分,移植特异模块得分为(29.90±5.16)分;多元线性回归结果显示,与生活质量总分存在一定相关关系的变量有:宗教信仰(t=2.005,P=0.047)以及是否发生移植相关并发症(t=-1.995,P=0.048)。结论异基因HSCT患者生活质量有待进一步提高,医护工作者应密切关注患者移植恢复期的生活质量,从多方面进行干预,以提高其生活质量。

Investigation of the Status and Influencing Factors of Quality of Life in Patients with Allogeneic Hematopoietic Stem Cell Transplantation

Objective To investigate the status of quality of life in patients with allogeneic hematopoietic stem cell transplantation (HSCT) and its influencing factors,to provide a basis for improving the quality of life.Methods By convenience sampling,129 cases were selected and investigated by using general scale and functional assessment of cancer therapy-bone marrow transplant (FACT-BMT).Results The total score of FACT-BMT was (107.40±20.22).The dimension score of physical well-being,social/family well-being,emotional well-being,functional well-being and BMT-specific concerns were (21.91±5.66),(20.78±5.00),(19.65±3.96),(15.15±7.07),(29.90±5.16),respectively.Multiple linear regression analysis showed that religious belief (t =2.005,P =0.047) and transplant-related complications (t =-1.995,P =0.048) were the influencing factors of quality of life.Conclusion The quality of life of patients with allogeneic HSCT remains to be improved,doctors and nurses should pay close attention to the quality of life of patients who in the recovery period,and to improve the quality of life from many aspects.
