妇产科手术后再次腹腔镜治疗异位妊娠84例

目的 探讨有妇产科手术史的异住妊娠患者腹腔镜治疗的可行性及临床价值。方法对84例有妇产科手术史的异住妊娠患者，采用经腹腔镜手术治疗，观察其效果。结果 有72例手术成功，成功率85．7％；12例中转开腹，占14．3％。12例中有5例合并子宫内膜异位症，6例合并慢性盆腔炎，有一次宫外孕手术史者9例．有两次官外孕手术史者4例。84例中尿路损伤2例。结论 腹腔镜治疗异位妊娠是一种理想的手术方式，但对有慢性盆腔炎、子宫内膜异住症及宫外孕手术史尤其是有两次宫外孕手术史者要做好中转开腹的准备。

84 Cases of Celioscopic Re - treatment of Ectopic Pregnancy After Gynaecologic Operation

Objective To inquire into the feasibility and clinical value of celioscopicre-treatment of ectopic pregnancy after gynaecologic operation.Methods To analyse the cIinical data of 84 patients of ectopic pregnancy who had undergone the celioscopic treatments during the period from Feb.2002 to May 2006 with the history of gynaecologic operation.Results 72 patients underwent successful operation with 85.7%,while 12 patients had to receive transit abdomen cut with 14.3%,among whom 5 patients have endometrium ectopic disease,6 patients have chronic pelvic infection and 9 patients have one operation of ectopic pregnancy and 4 patients have two times.Of all 84 patients,two patients have urathra lesion.Conclusion Celioscopic treatment of ectopic pregnancy is an ideal operational method but for the patients who have chronic pelvic infection,endometrinm ectopic diseases or one-time operation of ectopic pregnancy,esp.for those who have two times of operation of ectopic pregnancy,a full preparation should be made for the transit abdomen cut.
